If you have restrictions enabled in your Web browser software, certain parts of this survey may not display correctly. Some of the pages use javascript to check some critical entries before the form is submitted (for example, a date must be in a correct mm/dd/yyyy format so that a date of 23/8/06 would be flagged for an incorrect month and an incorrect (incomplete) year.) The application that allows you to select health history conditions also relies heavily on javascript.
Several of the screens have a "About this page" link near the top of the screen. Clicking on this link will call a pop-up page with information about the underlying screen. These pop-ups will have a "close" button on them.
You should make sure you have the latest version of whichever web browser you use. They can be downloaded at no cost and the latest versions have security features that protect you from harmful content. If you have an old computer running a non-supported operating system or very limited disk storage space left, then using a new version of a web browser may not be possible.
Click on the "Tools" icon (Version 7) or "Tools" on the menu bar (previous versions) - Select "Internet Options" - click on the "Security" tab - Click "Trusted Sites" to highlight it - Click on the "Sites" button and add http://www.LMHopkins.com to the list of trusted sites. If you don't do that, every page that has a pop-up window associated with it will display a yellow warning bar at the top which you have to right click and select "Allow blocked content". Javascript is enabled by default.
Click "Tools" on the menu bar - Select "Options" - select "Security Center" - select the "Current Site Rating" tab and click the little blue arrow next to "Your Trust Setting". In the next window check "I Trust This Site" and click "Done" . Netscape® is set to open pop-ups in a new tab instead of a pop-up frame.
Click "Tools" on the menu bar - Select "Options" - select "Web Features" - Make sure "Enable Javascript" is checked. If "Block Popup Windows" is checked, either uncheck it or click on "Allowed Sites" and add www.LMHopkins.com to the list of allowed sites.
